Sculptra is a poly-L-lactic acid (PLLA) injectable biostimulator manufactured by Galderma. PLLA is a synthetic polymer used for decades in medical devices (such as dissolvable sutures). When injected into the deep dermis or subcutaneous tissue, the PLLA microparticles trigger a controlled inflammatory response that signals fibroblasts to produce new collagen around the particles. The PLLA itself is gradually metabolised by the body over 18–24 months; the collagen it triggered remains, providing the volume effect. Result: gradual, naturalistic volume restoration that builds over months and lasts 2+ years.

Yes — Sculptra holds FDA Premarket Approval (PMA, P030050), originally approved in 2004 for HIV-related facial lipoatrophy and extended in 2009 for cosmetic correction of shallow-to-deep nasolabial folds and other facial wrinkles. This is the more rigorous PMA approval pathway, not 510(k) clearance. Sculptra is also CE-marked for use in the EU and UK and is widely used in licensed aesthetic practice.

HA fillers (Juvederm, Restylane, Teosyal) work by physically displacing tissue with HA gel — the volume IS the gel. Sculptra works by stimulating your own collagen — the volume is YOU. Practical implications: HA fillers give immediate change; Sculptra builds over 4–6 weeks per session. HA fillers last 6–12 months; Sculptra results last 24+ months. HA fillers can be dissolved with hyaluronidase if needed; Sculptra cannot be quickly reversed. HA fillers feel like product under the skin; Sculptra-stimulated collagen feels like your own tissue because it IS your own tissue.

Sculptra is most effective in STRUCTURAL areas of the face that need overall volume restoration rather than spot-precise correction: temples (where the soft hollows of ageing develop), cheeks, jaw, and the lower face for full-face rejuvenation. It is also widely used off-face for the neck, décolletage, hands, and for non-surgical buttock augmentation. Sculptra is NOT used in the lips, NOT in the tear trough/under-eye area, and NOT for very superficial fine-line correction.

A standard course is 2–3 sessions spaced 4–6 weeks apart, with each session adding a layer of collagen stimulation. More significantly volume-depleted faces may need 4 sessions. The full course is what delivers the substantial result — single sessions can underwhelm because the visible effect builds over months. Maintenance is then typically a single session every 18–24 months once the initial course is complete.

PLLA microparticles must be evenly distributed in the tissue immediately after injection so the body's repair response can integrate them smoothly. If clumps form before integration, nodules can develop — small, sometimes-firm bumps under the skin that can persist for months. The standard aftercare rule is "5-5-5": massage the treated area for 5 minutes, 5 times per day, for 5 days. This is not optional aftercare; it is part of the safety protocol. Your practitioner will demonstrate the technique and provide written guidance.

Sculptra cannot be quickly reversed with an enzyme the way HA filler can. The PLLA microparticles are gradually metabolised over 18–24 months; the collagen triggered by the treatment integrates with your own tissue. If a nodule forms in a specific spot, focused steroid injections or other targeted interventions may be used, but these are not the same as the rapid reversibility of HA filler. This is one reason Sculptra is reserved for experienced injectors and for patients with clear, longer-term goals — conservative dosing and patient selection are critical.

Common combinations: HA fillers for areas where Sculptra is not appropriate (lips, tear troughs); anti-wrinkle injections for dynamic-line areas; Morpheus8 or MOXI for skin quality and texture refinement; skin boosters such as polynucleotides for additional hydration.
